Alas, this is not the problem. I have a theory that it is in some
Headers settings, but the problem seems to be buried in a third party
module that is part of the web app.

ideas as to what I should be looking at to fix this. Any particular
behaviour on web pages you are trying to proxy that might break the
reverse proxy?

I suspect you have "UseCanonicalName On" elsewhere in your config.
Set it to Off and see if that helps.
