You question is specific to TYPO3 and I believe you would get better
support on their list. We run a few TYPO3 site and use the following
rewrite rule to support what you refer to as "speaking urls".
RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteCond %{REQUEST_FILENAME} !-l
RewriteRule (\.html|/)$ /index.php
