Exchanges that I am aware of:
PAIX - PaloAlto
LAIIX - Los Angeles
6TAP - Chicago
NSPIXP6 - Tokyo
and I think Amsterdam and perhaps one more exchange in Europe
support IPv6
Standard exchange switch fabrics work just fine.
