You usually end up having to join the developers programs of your "favorite" carrier and then getting permission through the developed channels.  I know that here in Canada Telus Mobility, for example, has a development SEMA SMSC in Toronto.  AT&T Wireless, Verizon and Nextel all have similar developer programs.
