I don't see any problems other than sqlbox (smsbox? bearerbox?) is not fetching dlr_url (I don't see any requests to webserver in logs). What bit are you talking about?
Does anyone have this stuff (sqlbox+dlr_url+dlr store=mysql with dlr status updating properly) running?
