i use standalone sqlbox,

for detailed experience you'd have to read the soure:

the mailing list have several posting re sqlbox, so just do detailed search for.

i consider stable
http://www.kannel.org/~mconte/sqlbox/sqlbox-standalone.tar.gz
as it was tested on high load MO/MT traffic.

we serve via kannel, sqlbox and pgsql high load MO/MT SMS services,
as well web based high volume bSMS service based on the same scheme.
