the proxy should reply with ACK only to negative replies of INVITE 
(hop-by-hop ACK) - for 200OK, the ACK is within DIALOG and should be 
generated by the caller (the 3-way handshake to initiate the call).

The proxy must propagate the BYE (no reply to BYE) and ACKs for 200ok.
