I think it actually is related to you building it with docker 1.12 and
pushing it using that client, there is a known manifest compatibility issue
which prevents openshift from pulling images pushed with docker 1.10+.

There are two ways you can work around this for now:
1) rebuild/push the image using docker 1.9

or

2) instead of using an imagestream in your buildconfig, use a DockerImage
reference and just specify the pull spec (
docker.io/jtslear/phoenix-builder:latest)
