Nevermind, the problem was with X-Lite softphone After receiving first 503 error it stops sending further requests for about one minute and report Service Unavailable error (very misleading, the error looks exactly like when it receives real 503)
No problem with OpenSIPS itself at all Thanks for your help and sorry for bothering
