Is this simultaneous sending the cause for the issue? Just thinking aloud. It is happening at only one installation and never in lab systems, which makes me think if it is a timing issue.

Two messages are sent simultaneously to the same destination. The first one succeeds and second one fails. I will check on connection tracking. You have asked me if it is off?
