It means Policyd is not getting a policy request in the RCPT stage, you
need to check policy in the RCPT stage to catch the recipients of the mail.

Add the following Postfix config...
check_policy_service inet:127.0.0.1:10031
in BOTH smtpd_recipient_restrictions and smtpd_end_of_data_restrictions.
