The minimum details needed to look into something like this are listed here:
http://wiki.policyd.org/support (refer to "full policyd log")

Without knowing what version of policyd you're running (output on one of the 
first few log lines), its pretty much impossible to advise.

The best would be to use the latest git and see if the problem persists.
