I'm using RoundCube 0.1.1. Some of the folders would be listed but  
there wouldn't be a subscribe checkbox in front of them. These were  
already mailboxes that I had available in that account so there was no  
need to 'trick' RoundCube by having a an email client like Thunderbird  
create them.

Setting create_default_folders seems to have solved the issue.
