I think jsf tags should not appear within the f:verbatim tags. Here's something that worked for me.

<h:panelGroup rendered="#{qoReviewForm.document != null}">
            <f:verbatim><tr>
                <td align="right"></f:verbatim>
                    <h:outputText value="#{qoReviewForm.document.title}" />
                <f:verbatim></td>
            </tr></f:verbatim>
 </h:panelGroup>
