I have resolved this issue. The blank tiles were created because I had designated an incorrect original projection in my mapfile's LAYER > PROJECTION parameter. I had thought the shapefile's projection was EPSG:4326 when it was actually EPSG:26910 (NAD 83 UTM ZONE 10N).
