Please check out http://documentation.openoffice.org for manuals and
HOW-TOs. Perhaps they will help you develope what you need, particularly
the templates and those on ooextras.org.

I suggest the docs so you can get an idea of how to build your forms and
implement a database and interface it with OOo.
