Which USA do I use to fill in country? There are sevral with a - between them.

In the grand scheme of things it doesn't matter. You aren't actually picking your country, you are picking the place that you are going to download from. The reason you chose, is that the close you are to the dl site, the faster it should come. You won't always notice an significant difference, but faster is usually better.
If you can decipher a location out of the names (it is tough sometimes) pick the one closest to you, otherwise, any US site should be fine.
