Adobe Acrobat Standard or Professional will create PDF's with live fields. AFAIK, there's no open source program which will produce PDF's with live fields.

If your recipients have OOo, send'em the .sxw with the fields. But if you're looking for something compatible with Reader, then Acrobat is all there is.
