It turned out to be the page style formatting - not the TOC.  I don't know
why, but even though the pages were numbered with Roman numerals (using
Insert--> manual break --> page-style=front matter), the page numbering
format had remained Arabic.  Modifying the page numbering format fixed it.
But your template made me re-think the problem.  Thanks!
