This question does seem to have an answer in the FAQs.

Some embedded user fields that include equations for auto-incrementing are simply converted to text for OOo version 1.1.4 -> MS Office. This appears to be working in the beta. There might also be some issues with respect to certain complex things that you can do with a TOC. OOo might support some things that that MS Office does not, so they can not transfer over directly. I seem to remember that MS Word in the newest version may support some new graphics anchoring methods that may not be supported in OOo, but I am not certain. I just remember that MS Office kept loosing the graphics images and moving slowly so I had to edit the documents in OOo, because it actually did what it needed to do. I also had to fix some page number tom-foolery where somehow MS Office had inserted two copies of the page numbers and then placed the fields on top of each other. OOo was able to fix that with no problems...
So, there might be problems with very complex documents, but I wrote a book in OOo while exchanging files with MS Office users with no difficulties, complete with tables, pictures, and complex formatting.
