It's not an error, it's the way it's designed. From the Help files:

The IgnoreAllList entry cannot be selected and cannot be deleted. Only the words included as content can be deleted. This happens automatically each time that OpenOffice.org is closed.

If you want specific words that don't appear in the regular dictionary to be ignored all the time, then you have a couple of options. You can either add them to the default dictionary and/or create one or more custom dictionaries.

Adding a word to the default dictionary works well for things like your own last name, your company name, or the name of your home town which are likely to appear in many or most documents that your create.

Custom dictionaries are appropriate for words specific to particular field. In my case, I have a custom dictionary I call "tech" where I stick words and acronyms that have meaning within my technical field but are not a part of normal English.

The IgnoreList works well for items, particularly proper nouns, that are specific to a particular document.

An option I would like to see is the ability to turn off spell-checking for URLs. That just drives me batty.
