The Mail Merge Create wizard will create a .csv file in the
/user/database folder (or on Windows: C:\Documents and
Settings\<username>\Application Data\OpenOffice.org2\user\database).

Can you find that file and see if you can open it in OOo Calc (set the
delimiters for Tab)? If so, open it and resave it (again as a .csv)
using a different file name. Now open Writer and Tools|Mail Merge Wizard
and see if you can use that newly saved/named .csv as the MM data
source. If that is still not working & care to trust me with the .csv
file, you can email it to me directly (glgxg at sbcglobal.net) and I'll
be happy to take a look at it for you & see if I can figure out what is
wrong. I do not have Vista, but I can test on linux, WinXP, Win2K; OOo
3.0-dev, OOo 3.0Beta, OOo 2.4.1, StarOffice8 and 9, and IBM Lotus
Symphony plus a few other's that I'll not bother mentioning...
