Just wanted to clarify something here.
Under Microsoft Windows (XP, Vista) Base is able to perform this function in an MDB file, also (not personally tested so maybe should say SHOULD ) in the newer ACCDB files used by MS Access 2007.

These manipulations include;

creating / dropping tables
adding / dropping / altering fields in tables (not every field types is supported in Base)
adding / removing / editing indexes
adding / dropping relations
(again I haven't done this with ACCDB but have with MDB files)

Anyway - just wanted to let you know.
