I've searched the Help files and I can't figure out how it decides what
to include in the TOC. I think it is based on paragraph styles, but

Also, I don't want the header in the TOC to say "Table of Contents." I
want it to say just "Contents." I know I can manually edit the TOC, but
is there a preferences somewhere where I can set the text and its
formatting?
