I want to generate an index of keywords (and perhaps phrases) in a document. That's involved if done by hand,

Normally, you will define each word you want to include in the index. Go to Insert -> Indexes and Tables -> Entry, make sure you notice the option "Apply to all similar texts." More information here: http://wiki.services.openoffice.org/wiki/Documentation/OOoAuthors_User_Manual/Writer_Guide/Creating_an_alphabetic_index

and if automatic, one has to have heuristics to decide what to include. And an automatic index would have to have some words the author would prefer were in the index and conversely.

Well, you have to give the "heuristics", in the form of concordance file, which basically list of words you want to be included in the index. You can slowly build the concordance file using the manual tagging above, or create it from scratch. The format of concordance file is in the help file (just look for concordance), but this site have it from OOo 2.1: http://plan-b-for-openoffice.org/ooo-help/r2.1/en-US/MAC/writer/01/04120250
