If you look at the basic libraries included with the OO.o distribution
you will find
[OpenOffice.org Macros & Dialogs].FormWizard.GetConnection
I call call this routine 'well written' because of how it uses structure
and default UI handlers to handle the login dialog.
As for examples of insert statement - care to be any more specific as
there seems to me to be many such on the forums to choose from.
