Thanks for the response.
I almost always click the "X" or (ctrl-w) out of the window, and never
(file -> close). Most often click the "X" on the window. I will pay
more attention to that in the future. This problem seems almost
impossible to re-create on demand.

This is a great annoyance for will, plus it is not aiding me in
convincing my employees to switch to openoffice.org. This never
happened in OOo 2,2.4,3.0 - but in 3.1, this happens.
Any ideas?

Not really sure why that would happen under 3.1.
Let me ask you - if you open the form for data input and just close it
via the system close menu (click on the X in the upper right hand
corner for instance) then this asks you to save your changes?
