I thought that perhaps the document was "inheriting" problems
going back to 1996, when it was first written on an old version of
Word. So I saved the document as a text file, I then inspected the
text file using some of my programming tools and looked for any
'invisible' characters that might be other than conventional
whitespace. Found nothing unusual. Opened the text document in OO,
saved in ODF format - supposing that erradicated all traces of
  Word.

  Same problem!

I find that if I open a blank document, this process works just
fine! And to further test if I insert blank lines, to create a
second page, the increment works, page number is visible and can
  be moved, justified etc.

It is said that a Good Retreat is better than a Bad Stand, so I'm
going to take a little hiatus and thoroughly study the
documentation that was recommended to be in an earlier thread.
(PDF's on Formatting Pages, Styles, Templates et. al.)

I will then tackle the problem again and perhaps some of you who
have contacted me OTL in the past will hear from me OTL with a
propo$al. This is a critical issue for my wife and the most
difficult software problem I have **ever** run into. And I must
say that my wife asserts that this issue is trivial and easy to
  accomplish in Word!
