Which mail-program do you use? You should be able to right click on
the icon that represents the attachment and choose "save as" from the
context menu. 

If I open an attachment directly from within my mail-program then the
document gets some "incomprehensible" name and is temporarily safed
in the temp-directory of my computer. If you don't attach this
document to your reply, then I'm not surprised, that you're changes
aren't visible for the recipient. 

So, if you open an attachment directly from within your
email-program, be sure to do a "save as..." instead of "save" to be
able to specify, where your computer should save the document.
