Ian, the method I use is based on Avery business card stock (J8414) and the simplest method is to use a table-based template. I probably obtained mine from the Avery web site originally and it was quite likely an MS Word original with which OO.o had no difficulty. If you are in 'A4' country I'm more than willing to share; if not I suggest the aforesaid Avery site.
