The good news is: You're not alone. I've been writing up problems about
printing to HP4000 for a long time.
The bad news is: The developers do not consider envelope printing to be
a priority. I base this conclusion on
http://qa.openoffice.org/issues/show_bug.cgi?id=17466
(my 38454 was classified a duplicate) wherein the nature of the problem
is characterized as "Enhancement" and the projected completion date is
"OOo Later".
In response to one of my submittals, they did admit that there are
"problems with certain HP drivers and Win98."
Nobody who doesn't use an HP4000 can believe the problems are as bad as
reported and you will receive all sorts of replies telling you exactly
what to do to print envelopes. I've tried them all. Forget it.
My fix is to use window envelopes until the project decides to take the
issue seriously.
If I sound irritated about this topic, that's exactly right. It is a
major stumbling block for me and anyone else who isn't using a "throw
away" printer.
