Add your spreadsheet as data source, assuming you're using OOo1.1.x this can be done under Tools.Options.DataSources. Then go to File.New.Labels and in the dialog select as Database the data source you just defined, the sheet to use as Table, and proceed with the Database fields. If done, create the New Document and print it as form letter, you will be asked for it if you select File.Print. In the MailMerge dialog that follows specify the records to use.
