     It is all about Styles. The listings in the ToC have the heading 
styles that are selected in the Entries tab of the Insert Index/Table 
window. (Insert > Indexes and Tables > Indexes and Tables)
The ToC Heading Style is usually Contents Heading. That is why it is no 
usually included.
     By changing the style of the ToC Heading to one of the Heading 
Styles, it could be included in the ToC itself.
