Not at the top but in the Structure field and it's
there under XP but, under FC4 and I expect other linux distros, the font
used is not particularly good and looks like and F#. IIRC, there is an
issue on this but do not recall the number.
Unfortunately, the Structure line is poorly rendered (Windows XP with
1.9.125). I put arbitrary strings (AAA, BBB, etc.) in the white spaces.
This gave me a clue as to what is going on.
After playing with this, I now have the desired spaces between the
Heading Number and the text of the heading. Also, I now have hyperlinks
from the contents to the headings.

I'll play with the formatting in the Entries Tab to see what I can make
of it.
Like the manual says, put your cursor in the box preceding the field
with which you wish to start and click hyperlink. Repeat to put insert
hte hyperlink end and click all.

I have a TOC referencing three heading levels.
The first level leaves a space between the heading number and the text
of the heading. The other two do not.
Does anyone have any suggestions/
