OpenOffice.org version 2 just came out today, so whenever asking a question like this, it's a good idea to mention which version you have.
However, in this case, the two versions work alike. There is no "Header/Footer", but there is a "Header" and a "Footer". You just need to pick the one you want. They're exactly alike, apart from one being at the top of the page and the other at the bottom, so the Help treats them together.
