I did some further testing and even the document
created in ver 2 is displaying the same problem.

This only happens when you have several chapters in
your document (using the default Chapter Styles, Title
and Subtitle) 

AND

If you set the footnote to be counted PER CHAPTER,
compared to the default PER DOCUMENT.


Thank you for verifying. I am about to file this bug
and I hope that you'd help by voting this issue.
Although this is not *critical* and has a workaround,
it becomes tiring and burdensome in the long run.
