You might want to check out the very recent response to a similar question,

been  real solid on my understanding of page styles and how to use them myself,

Hi Pete,

OOoAuthors.org has some excellent guides  for using OpenOffice.org 2.0.

Regarding  your question specifically, I would apply the "First Page"
page style to the  first page, then modify that style so the header is
on.  Then I'd modify  the "Default" page style so that footers are on
(headers are already off by  default).

Here's how:

Let's tackle page 1 first...

- Bring  up the Stylist by pressing F11 or by clicking the "Styles and
Formatting"  button (looks like a hand on a blank page - it's at the far
left by  default)

- Select the Page Styles icon (4th from the left)

- With  the cursor on page 1, double-click the list entry "First Page".
You've just  applied the First Page style to page 1.

- Right-click the "First Page"  entry and choose "Modify..."

- On the Header tab check the box for  "Header on."  Click OK.

Now for all the other  pages...

Assuming the Stylist is still up and in the Page Styles  section,

- Right-click the Default page style entry and choose  "Modify..."

- On the Footer tab check the box for "Footer on."   Click OK.

You've now told Writer to put footers on all pages that are  styled as
Default.  That means all pages except for page 1, which we set  to the
First Page style.

Now that everything is set the way you wanted  it, save the document as
an OpenDocument Text Template  (.ott).
