You have to use Courier New 12 pt as font everywhere (including headings, titles, etc.) and set unit to Pica (1 pica = 12 pt) in Tools/Options/Writer/General/Measurement unit. Then rulers will be set in one character unit, so if you set margins to make text wide 60 pica, you will have what you need. Setting correct height of the page should be slightly easier.
