I agree that there are errors but perhaps your suggestions reflect poor
English usage as well.
The SURVEY is what the OOo admin. people are doing. The respondent is
completing a questionnaire, not a survey. (Also, you do not TAKE a
survey, you DO a survey)...
The choices should be consistent with the stem question.
e.g.
1) Is this the first time you are completing an OpenOffice.org
registration questionnaire?
Yes - this the first time I am completing an OpenOffice.org registration
questionnaire.
The tense is clumsy to our understanding but there are many respondents
for whom English is not their first language but who are answering in
English. The use of the present (continuous) tense is easier to
comprehend than the more usual implied past tense - Is this the first
questionnaire you _have completed_ etc.
