Well, out of curiosity, with OOo 2.0.0 (on Linux) I did the following:

I started xv, saved the default image (xv logo) into a postscript file.
Then I ran ps2epsi to create an EPSI image (with preview).
I started OOo, new text document, Insert->Image->From file.

bug 1:
 No preview image, just a box stating the file info. The EPSI file
 *does* contain a preview bitmap but it is not displayed. This bug was
 reported during the beta test that was run by Sun (for SO8, which by
 the way, is still not available in Australia for less than 25 seat
 installations...)

Then I went File->Export to PDF

bug 2:
 The generated PDF file is perfect WYSIWYG: it contains a frame that
 contains the textual file info, exactly the same way as was presented
 on the screen.

If instead of exporting it I print to a postscript file, the image is
there. So it is not Rick Smegal's local issue, it is an OOo problem
(that has been reported about half a year ago). Maybe OOo handles some
EPSI files corretly, some others incorrectly. The above mentioned way
of creating an example is fairly simple, xv and ps2epsi are quite 
readily available.
