1. Right-click on the TOC and select Edit Index/Table
2. Click on the Index/Table tab
3. In the Create From area, tick the Additional Styles checkbox
4. Click on the ... button
5. Select Index Heading
6. Click on the >> button till you get it at the heading level you
want (eg if you want it looking the same as Heading 1 entries in
the TOC, click the >> button once)

For whatever reason, the created "Alphabetical Index" that titles the
table in the back of the brochure does not show up in the created
"Table of Contents" in the front of the brochure.
Could not find this listed as an issue.
Any information?
