Great rates starting at 1¢/min.--- Begin Message ---Lets put it this way, how do I enter data into the form?If I OPEN the form, the form fields are read only so I cannot enter. If I EDIT the form, this only makes structural changes. This does not allow me to enter data either. If I try to add a record in the TABLE, I cannot add a new record in dataview and enter text.

I saved your database file and opened it. Then I right clicked
the form Contact and then Edit. What I got is shown in the attached
PNG snapshot.
Note the "green handles" surrounding the first field in the form.
I left clicked the first field to show that it was in Edit mode. Also
note that the Styles and Formating window is open to the left. That
is only visible when the form is in Edit mode.
So, it is most likely that something in your operating system
that is causing the file to be read only. It is possible that the
database file has been changed to a Read Only file. This would be
part of the properties of the file. Or there could be a permissions
problem.
