At the risk of inviting, "that's the wrong way", I have always found a
home-made template to be the easiest way. Populate it with appropriate
user data fields and then use it to create a savable document.
I use an A4 sized template for Avery 2 * 5 sheets, to which you are
welcome, or I would be happy to knock up a Letter sized one if you wish?
