Using OO v2.0.3 how do I prevent a merge document leaving blank
lines where some of the fields in the database (addresses) are

You could make two copies of the mergedocument. One with an
address-line, one without. Then when you merge the document and the
data instruct OOo to select records with or without an address.
Downside: if anything changes in the document you must change it
twice.
A better approach is the use of hidden paragraphs.
Set as condition '<name database>.<name table>.<name field> EQ ""'
(without the single qoutes, without the brackets).
