I'm also experiencing this on KDE, on my Kubuntu box. It started when
I set GTK themes to mimic QT themes. Being GTK, Open Office was
affected, but it's the only app that can't seem to cope. Not only are
the icons missing in my case, but also the menu items (until I hover
over them). Included screen shot of Impress with 'hidden' icons.
i've set my GTK styles back to bluecurve and it seems to be ok now
