I agree it's quite an annoyance, but I have no idea why this happens
or how it could be fixed, sorry. With respect to this, however:

the answer is yes, there are mail readers smart enough to keep each
message "in-thread" with the message they reply to, regardless of the
subject(s).

One is the one I'm using now, mutt. In general, however, any client
which can properly recognize and generate the "In-Reply-To:" header
will do this, so that's the feature you want to look for if you need
to keep threads together. If I remember correctly Thunderbird (for
linux, windows and maybe Mac too) can do it.
