On the Save As ... dialog there is a checkbox that says something like automatic filename extension. If you check this, then you don't have to add the .doc yourself but OOo will always add the extension according to the fileformat you're saving under.

Simply saving the file with ".doc" at the end of the name is *not* sufficient. If you do this then the document will be readable by MS Word. 2. When you sent the document as an attachment without saving it, what exactly do you mean? What you need to do is use the File>Send>E-mail as Microsoft Word ... menu.
