I have posted the original .odt form, the Junicode fonts and the New
Database.odb file needed in order to open the .odt file, and the
exported PDF file to my personal web page at:

I tried to reproduce the behavior, but if I'm not wrong the data base
does not contain the table needed (it complains about a non existent
'Test_Table' ).
At this point I think there is a bug in the PDF export from Writer in
OOo 2.03. I sure hope someone can figure out what is going wrong. I
would also seriously appreciate it if someone with 2.04 would open
the .odt file and export to PDF, then tell me if the Junicode font got
embedded in the controls in the PDF file. (It embeds fine in the rest
of the document with 2.03, just not in the controls.) If it does embed
for the controls with OOo 2.04 it may make it worth my while to upgrade
my Ubuntu amd-64 Dapper to Edgy. Version 2.04 cannot (apparently) be
installed in Dapper, but it is the default in Edgy.
I was not able to test completely the whole thing (see my comment
above), but I had a look at the OOo source code and it truly seems that
the font in the combo box/list box items if forced to Helvetica.
Confirmation from the OOo developer folks pending, though.
If you had time, could you work with me to build a test case with the
good work you've done so far, so we can either open a new issue or add
the test to an existing one?
You can send the files to me privately, if you like; as soon as we have
figured out what to do I'll post the list the resulting issue number.
