I had exactly the same problem. The only way of solving this for me was
to uninstall OO.o and to install JRE from Sun and OO.o from the
OpenOffice.org sites directly (not the Debian version where there is a
Java problem).

Setup a subform (none)
Arrange Controls (Columnar - Labels on top)

Apply Styles (water)
Set name (DVD) & work with form
