I still don't understand why it would be necessary to add blank pages in a pdf, especially AFTER all the indexing is done. It doesn't make much sense. Fortunately, ps2pdf seems to work in a logical fashion and not screw up the pagination. When I export as .pdf there are no changes that I am aware of. The pages listed in the TOC are the actual pages before and after the export to .pdf.
